Thanks for all the replies.  I do take off the little cap that is meant for a hose attachment.  However, I'm rarely close enough to put my foot on the part that goes into the sewer pipe and it won't screw in, so....  the pump causes the end to pop out of the pipe.  I like the suggestion of the ball valve.  I'll have to ask my husband to do "one more thing" to the almost new RV.  LOL. 

If you look at the picture above posted by Will Lloyd, you remove the whole black end cover, not just the hose thread cap.

Look at drawing 3 and 5 on this website, 3 shows the nozzle with the black end cap removed.   5 shows the end cap that you take off (unscrew to remove) that also has the garden hose cap.

Not to be repetitive, but I am.  Just remove the entire gray cap so you have a 3" discharge, this eliminates the pressure pushing the hose out of the hole.  Only use garden hose option if there is a need to extend the reach of the permanently attached hose.

For those that think I'm trying to push everything through the tiny hose - no, I've never done that.  I always remove the end cap so I have the 3" opening.  However, the pump pressure is still strong enough to pop the hose out of the sewer hole.  However, my husband has come up with a solution.  He made a remote control "thingy" that turns the  pump off/on with the click of a button on a hand held remote.  I can be standing on the plastic thing that fits into the hole and turn on the pump from there.  The original pump switch will still work, but isn't needed if I want to use the remote.  In case the remote switch gets pushed accidentally, there is also a toggle switch on the little box that he built that turns it off.  I'll take pictures later although I have no idea how he built it.

If I can figure out how to do it - here are the pictures.  First picture is the remote control box that is wired to the pump.  Second picture is the remote.
